Apply Online

Applying online is easy. In fact, we prefer to receive grant applications through our Web site as it accelerates the processing time. If you are unable to complete an online application, please contact our director of grants management at (303) 953-3672 or grants@ColoradoHealth.org to make alternative arrangements.

Step 1:  Read the Grant Guidelines and Frequently Asked Questions
The Grant Guidelines explain each section of the application and describe the types of questions you will be asked to answer, while the FAQ further expands on the process. If you have additional questions that are not answered in these documents, please Contact Us.

Step 2:  Begin your online application

Online Grant Application - Start

You will be prompted to enter your tax payer identification number. If your organization does not have a tax payer identification number, you may enter "0." At first the system will state the following: "The Tax ID that you entered was not found in the IRS database. Please check your entry. If it is correct, click "OK" to begin filling out your application form." Then click "OK" to begin your application.

At any point in the process you can click "Save and Finish Later."  The first time you do this, you will be directed to "Please Sign In." If you are creating a new account, click "I am a new online applicant," enter your e-mail address and create a password. Write down your password for future reference. If you previously created an account for your application, return to the online application and click "I am a returning online applicant" and enter your e-mail address and password.

You then can review the information you've provided so far and make changes if needed. You can also print your application at any point by clicking "printer friendly version."

Step 3:  Make sure all necessary attachments are included
The online application will prompt you to attach each required item. You also may refer to the Attachment Checklist to make sure that you have included all necessary materials. If you have any questions about the attachments, please Contact Us.

Step 4:  Review and submit your online application
Once you are finished, click "Review and Submit." You will have one more opportunity to review your materials before submitting your application. If any elements are missing, you will be prompted to complete the relevant section before moving forward.

When you click "Submit," you will receive an e-mail confirmation that we have received your application. Please make sure you enter your e-mail address accurately on your application. If you do not receive an e-mail confirmation, please call our director of grants management at (303) 953-3672.

NOTE: If you are submitting an application online, please do not mail a duplicate hard copy or any additional attachments or information. Program officers will contact you if they have questions or need additional information.

Review Process
After the application has been assigned to the appropriate team for review, you will receive another e-mail notifying you of the program officer who will serve as your primary point of contact. Your program officer will contact you to discuss your proposal and follow-up with any additional questions. Program officers will occasionally schedule a site visit to meet you in person and further discuss proposed activities. 

The review process takes approximately four months. You will be notified as soon as a decision is reached. At any point during the process, please do not hesitate to contact your program officer with questions or concerns.
