Model Grants

Following are just three examples of how we work with grantee partners to achieve measurable results toward improving access to the key components of Health Coverage:

Family Resource Centers
Each year, more than 70,000 Coloradans visit Family Resource Centers to access vital resources, such as health care, parenting classes, adult literacy programs, food and clothing, job skills training and lunches for seniors. The Foundation has granted more than $4.5 million to Colorado's Family Resource Centers to help them improve access to health care for uninsured and underinsured individuals served by the 24 centers throughout the state.

Colorado Children's Healthcare Access Program
The Colorado Children's Healthcare Access Program works to ensure that every child enrolled in Medicaid and CHP+ receives comprehensive health care in a medical home. We granted the Colorado Children's Healthcare Access Program $396,000 to support their efforts to enable and encourage private practitioners to provide medical homes for 140,000 children from low-income families.

Denver Health Foundation
The Denver Health Foundation supports Denver Health, the Rocky Mountain region's safety-net hospital system. As part of its mission to advance the health and well-being of area residents, Denver Health oversees school-based health centers (SBHCs) in 12 Denver Public Schools, which provide needed medical care, as well as outreach to and enrollment of eligible children in Medicaid/CHP+. We granted more than $2 million to the Denver Health Foundation to support the expansion of metro Denver SBHCs.
