
Making a Measurable Difference

Think of the Measurable Results as an instrument or metrics that can be used to gauge progress toward our three outcomes – Healthy Living, Health Coverage and Health Care. When applying for funding, grantees are asked which result(s) their work targets and how their activities, if successful, will have a meaningful impact.
Grantmaking decisions are based on how the proposal aligns with our goals and the ability to produce one or more of the Foundation’s Measurable Results.
We also will invest in organizations and partnerships that educate key decision-makers, develop leaders, build coalitions and inform the public about health policy issues if the proposal includes clear and tangible strategies to impact one or more of our Measurable Results.
Outcomes & Measurable Results
Outcome: All Coloradans have access to the components of Healthy Living —nutritious food, physical activity and disease management tools—because these are key to staying healthy.
Measurable Results:
>> Decrease Colorado’s obesity rate:
- Increase number of children and adults who engage in moderate or vigorous physical activity
- Increase number of children and adults who eat adequate amounts of fruits and vegetables daily
- Increase number of children who receive healthy meals at school and have access to healthy vending choices
- Increase number of underserved Coloradans who have convenient access to recreational exercise and fruits and vegetables
>> Increase number of parents who are educated on child development, nutrition and preventive health care
>> Increase number of Coloradans who are educated on chronic disease management
Outcome: All Coloradans have Health Coverage because people who do are more likely to receive necessary care.
Measurable Results:
>> Increase number of children and adults who have adequate health coverage
>> Increase enrollment of eligible Coloradans in Medicaid and the Child Health Plan Plus (CHP+)
Outcome: All Coloradans receive quality, coordinated Health Care because those who do are healthier.
Measurable Results:
>> Increase number of underserved Coloradans who receive integrated care
>> Increase number of underserved Coloradans who regularly receive primary, mental and oral health care
>> Increase number of medical professionals who serve underserved Coloradans
>> Increase number of patients who receive evidence-based care for chronic disease
