
Our Guidelines

Through our grantmaking, we seek partners who will help make Colorado the healthiest state in the nation by working toward the following outcomes:
- All Coloradans have access to the components of Healthy Living —nutritious food, physical activity and disease management tools—because these are key to staying healthy.
- All Coloradans have Health Coverage because people who do are more likely to receive necessary care.
- All Coloradans receive quality, coordinated Health Care because those who do are healthier.
Our funding focuses on populations whose health needs are not being met due to income, age, race, ethnicity, geography or other reasons.
To be accountable, we have committed to evaluating and measuring our progress. We have established 12 Measurable Results that align with the three outcomes listed above. Grants will be awarded to organizations that can demonstrate the capacity to achieve these results.
We will regularly report our progress, sharing both successes and projects that didn’t go as planned. We will use the lessons learned from the evaluations to adapt our grantmaking and hope our grantees can also benefit from this important tool.
