School-Based Health Care Resources

The Colorado Health Foundation’s School-Based Health Care Initiative calls on communities to draw upon many resources to build successful models of school-based health care. This Resource Guide points program applicants to a variety of resources that can support effective school-based health care services. The Foundation also recognizes that the best and most helpful technical assistance often comes from informed, experienced individuals or organizations that have “walked the walk.” With that in mind, the Colorado Health Foundation will partner with the Colorado Association for School-Based Health Care to provide technical assistance and other resources to assist with the application process.

Colorado Resources

The Colorado Association for School-Based Health Care (CASBHC) is working closely with the Colorado Health Foundation to provide technical assistance and support to communities exploring how they might initiate school-based health care under this initiative. The CASBHC annual conference and training sessions are all designed to provide technical assistance on key topics.  

Colorado Connections for Healthy Schools 

The Colorado Department of Public Health and the Environment (CDPHE) has been involved in the development of school-based health centers since 1994. Information on the state’s school-based health center program and resources related to best practices, state funding and quality standards are found on the Web site. Other CDPHE resources include:

National Resources

The Center for Health and Health Care in Schools, a research and policy center located at George Washington University in Washington, D.C., maintains a Web site with resources devoted to school-based health centersschool mental health programsschool-based dental health programs and school-connected programs targeting childhood overweight and obesity.

The National Assembly on School-Based Health Care with members from school-based health centers across the United States has launched the SBHC Road Map, an online Web tool that provides current resources and technical assistance to aid SBHC administrators from the initial planning phase through all phases of an SBHC’s operation. The NASBHC Web site also includes helpful sections on quality improvement and community advocacy.

Other Resources

Behavior change 
The Center for Learning Excellence maintains an evidence-based program database with information about programs recommended by research-oriented government agencies, nonprofit agencies and independent publications. The programs featured demonstrate youth behavior change. 

Community assessment
Find Youth Info, a federal government site, is an interactive tool that focuses on forming community partnerships and youth-adult partnerships as well as conducting community assessments. It includes a section on conducting a community assessment that provides a mapping tool with youth-related census data and location maps. Communities can also identify the location of federal programs that serve youth. It will also help identify what additional resources may be needed and the best location for providing services. Other resources include information on technical assistance and funding, a summary of youth risk and protective factors and a bibliography for research on risk and protective factors for youth.

Data
A Guide to Resources for Creating, Locating, and Using Child and Youth Indicator Data, created by Child Trends and KIDS COUNT. This guide provides brief descriptions of data and information resources available to measure child and youth well-being.  

Healthy child development
Child Trends’ Lifecourse Interventions to Nurture Kids Successfully (LINKS) describes social (non-medical) research-based programs designed to enhance child development.

Integrated Care in the School-Based Setting

According to the Colorado Association for School-Based Health Care, "integrated school health services" means comprehensive, coordinated, continuous and age-appropriate physical, behavioral and oral health services, provided by a multidisciplinary team to students while they are in school, using a process of care that includes direct delivery, co-management, and referral. Colorado has several examples of integrated school-based health care programs. This webinar explores how integrated care works in the school-based setting and strategies for addressing clinical and operational issues associated with delivering integrated health services in schools.
