
Grant Examples: Ensure Adequate and Affordable Coverage
Examples of grants in this Funding Strategy include:
PROMETHEUS — The Foundation provided a grant to Health Care Incentives Improvement Institute Inc. (HCI3) to support a PROMETHEUS pilot program in Colorado. PROMETHEUS is a new compensation approach that recognizes and rewards health professionals who meet evidence-based performance measures.
Larimer County Department of Human Services – A two-year project increases the percentage of people eligible for long-term care Medicaid that get enrolled in coverage by providing hands-on assistance for those with limited abilities to complete the process. The project is a collaboration of Adult Resources Care and Help and Options for Long-term Care, both programs within the department.
Boomers Leading Change in Health – This three-year pilot initiative recruits, trains and places a corps of laypeople – ages 50+ – as health system navigators, community health workers and advocates for improving health care for all.
Colorado Health Institute – Foundation funding enables this organization to continue research and analysis regarding health coverage and access, the safety net system, policy analysis and promising new models of care.
