Managing Your Grant

This page outlines key guidance on communicating about your funding, submitting required reports and requesting amendments—so you can focus on what matters most: making an impact in your community.

Grant Reports and Amendments

Grant reports and amendments help ensure your work stays aligned with the goals outlined in your agreement. In this section, you’ll find what to know about submitting required reports and requesting changes through our grantee portal.

The Colorado Health Foundation uses grant reports to help us better understand the impact of the work being funded. As outlined in the grant agreement, all grantees are required to submit reports, and some may have extra requirements. Be sure to review the grant agreement for your specific reporting schedule and submit all reports through our grantee portal. 

Email reminders will be sent from The Foundation approximately 8 weeks and 4 weeks before your report is due and will include links to the grantee portal with instructions.  

After logging into the grantee portal, go to the Upcoming Reports section to find your report. You have three options to complete it: 

  1. submit a streamlined written report
  2. upload a previously prepared report
  3. schedule a call for a brief verbal report 

No matter which reporting option you choose, these are the questions that need to be answered. Please note that grantees with an Expenditure Responsibility grant are required to submit a written report. 

When you’re ready, select the Edit button in the top right corner of the report and follow the prompts to choose your reporting option. After selecting and completing the prompts for your report whether written or verbal, select the Save and Close button and then Submit. You will receive an automated email within one business day confirming that your report has been received.

For various reasons, you may need to request a grant amendment, like extending the grant period or updating milestones to match your work. To do this, log in to the grantee portal and go to the Request an Amendment section. If you’re new to the system, register on the portal landing page and we’ll set up your account. 

After you have completed and saved the draft amendment, click Submit at the bottom of the page. Your request will then be reviewed by staff, and you will be notified of the outcome.

Communicating About Your Grant

Communicating about funding that you receive from us is a consideration for each organization, especially if you are a grantee hoping to maximize the impact of your work and mission.  

We do not regularly announce grants, sponsorships or donations unless it is for a specific program, such as the Nonprofit Sabbatical program. However, we encourage you to communicate about funding received if it is helpful to your mission.  

Below are guidelines for any communications related to a grant (including sabbatical grants), sponsorship or donation that we award.  

Grant Awards: Once a grant agreement is finalized, you may consider letting your audience know about it. There are a variety of ways to announce your grant award, including through news releases, articles, email and social media. We do not need to review content that you produce related to a grant you receive. If you’re interested in including a quote from the Foundation in your press release, please contact your program officer to discuss. 

Sponsorships: Our sponsorship request process provides information to us about marketing needs related to an award. Our staff work together with the organization who receives a sponsorship award to ensure all communications needs are met. Sponsorship awardees will receive information about who to contact. In the meantime, feel free to use the guidelines and files below as needed to represent our sponsorship.  

Donations: Donation awards are sent directly to organizations for various reasons, including crisis situations and as designated gifts from our staff or board. Recipients of donations are welcome to recognize us if they wish using the guidelines and files below. 

The name of our organization is “The Colorado Health Foundation.” Please reference us as “The Colorado Health Foundation” on first reference. For any subsequent references, you may reference us as “CHF” or “the Foundation.” 

The Foundation’s high-resolution, four-color logo can be downloaded for your use. If you’re using our logo on a dark or patterned background, please download our white logo. The logo is available in EPS, JPEG and PNG formats and in three different orientations – centered, horizontal and vertical, in 300px and 1300px size options. You’re welcome to use whatever format and orientation that works best for you. Please note: our logo should not be altered or distorted.

Your organization may want to share the news of your new grant award with local or statewide media outlets. This press release template is designed to help you draft that communication. 

The Foundation is active on the following social media platforms. We encourage you to follow us and tag us in posts related to your grant award using hashtag #COHealth.
