Chain Breakers Hope Makers (CBHM) - Western Colorado Recovery Network

Build a strong, sustainable ?peer-led? recovery support network by strengthening and aligning existing resources in the region, with a focus on serving vulnerable populations including individuals experiencing homelessness, leaving treatment, those being released from incarceration, and the Hispanic community. Integrate this network into existing health care and social services such as community health centers, workforce development programs, and family services agencies.
