Linguistically and Culturally Responsive Navigation for Non-English, Non-Spanish (NENS) Speaking Patients

Expand the work to screen NENS patients 0-5 years, through the use of linguistically and culturally responsive multilingual care navigation, by 1) ensuring NENS children and adults screened and referred for specialty care and services are able to access that care, and 2) connecting NENS child and adult patients without a medical home and present themselves to the Denver Health Emergency Department and urgent care with permanent primary care.
