Project Description

Lead a community-wide collaboration to expand outreach and enrollment over two years to a minimum of 9,200 individuals eligible for Medicaid, Child Health Plan Plus (CHP+), or Connect for Health. This effort targets Pueblo County and surrounding areas served by Pueblo County providers and other health and human services organizations. Pueblo StepUp is uniquely positioned to successfully lead this unprecedented collaboration with sponsorship from Centura Health.
