Project Description
Develop equity goals and a Roadmap for CCHN and the 20 Community Health Centers (CHCs) in Colorado that will serve as a guide for CCHN and the CHCs to begin or strengthen the related organization's equity journey. The work will include developing equity goals, building the Roadmap itself and continuous discussion, training and education with CHC leaders and staff on systemic racism, how to use the Roadmap and moving the work forward.
