The Colorado Health Foundation believes that health is a basic human right. This report encompasses a relentless pursuit of health and health equity, and it takes engagement on the ground to understand what is happening in and impacting communities across our state. At the Foundation, community engagement is both a process and an outcome. The Community Engagement IMPACT Practice Model provides the guiding principles that represent the actions and behaviors that program staff should embody.
