Expand programming to include Community Health Workers (CHW). CHWs will meet one-on-one with individuals, as well as, groups to provide assessment, education regarding risk factors and healthy behaviors, as well as, host educational and interactive workshops and classes. This addition will help decrease barriers for Summit County's most vulnerable populations.
