Project Description
Enable the Community Assistance Referral and Education Services (CARES) Behavioral Health Team to continue finding emergency services super-utilizers appropriate community resources for their ongoing health through increasing the team's caseload by 60 patients. With this support, CARES could provide much needed additional services for the 76% of CARES EMS users with behavioral health complaints.
