Project Description
Support expanded telehealth capabilities at the Montbello Family Health Center (MFHC), the Lowry Family Health Center (LFHC), and the Sloan?s Lake Health Center (SLHC). These Federally-Qualified Health Centers care for a rapidly growing and economically-disadvantaged population. Funding will equip the community with access to telehealth services including video visits, remote patient monitoring, and even specialty care within the primary care setting.
