Advancing Team-Based Care at San Luis Valley Health through Technical Assistance

Enhance the delivery of health care services within the four Primary Care clinics, utilizing the Team Based Care model to ensure San Luis Valley Health's (SLVH) three Rural Health Clinics, along with its provider-based primary care clinic, are universal in their approach to providing team-based care. SLVH will achieve this only by ensuring that health equity, accessibility and cultural responsiveness are at the forefront of all service delivery enhancements.
