Development of a Regional Center of Excellence in Care Coordination for the Eight Counties in Southwest Colorado

Plan and develop a regional Center of Excellence for Care Coordination, as a way to bring multiple care coordination initiatives being implemented in San Miguel, Ouray, Montezuma, Dolores, Archuleta, and La Plata Counties together for: support and resources; program and outcomes evaluation for individual care coordinators, practices, and communities; defining care coordination models; improving consistency in training and scope of work; and improving and increasing the use of evidence-based practices.
