Project Description
Provide ongoing case management to individuals experiencing homelessness that addresses mental health, recovery and long term stability. TRC serves a population that is most impacted by mental health and substance use and has the least access to help. Through street and shelter outreach, TRC helps individuals connect with resources, find purpose, build ongoing healthy relationships and achieve long term stability.
