Pay for Success Pilot to Improve Health and Access for Coloradans

Support health care and support service provision for 300 chronically homeless adults as part of a supportive housing pilot and social impact investment transaction with the City of Denver. The work will focus on improving care coordination and maximizing Medicaid reimbursement, leading to program sustainability and a replicable model for other providers across the state.
