Project Description
Redesign a building in Fleming, CO to create a proper clinic and provide primary and specialty care services 5 days a week, increasing access to care for over 1,500 unique individuals. Haxtun Hospital District currently owns the building and conducts primary care clinic one half day per week in an attempt to meet the primary care and family medicine needs of the community.
