Increasing New Americans' Access to Primary Care

This project will provide refugees and refugee-like populations in the Denver metro area with equitable access to primary care through the provision of health education, home health visits, transportation assistance and medical case management. It will also assess key variables in delivery of project elements to determine ideal content, dosage, sequencing and timing of profject activities.
