Project Description
Rebuild and strengthen Team-Based Care (TBC) foundations for Every Child Pediatrics' traditional clinics and four school-based health centers in Aurora Public Schools. With technical assistance support, develop effective TBC models that allow expansion of access to care, increased care delivery efficiencies, effectively address SDoH and increased job satisfaction.
