Project Description
Introduce a model for community care coordination serving 11 rural and frontier eastern Colorado counties. This model would be to partner with clinics to provide clinical workforce training for care coordination, for home visits, attend clinic appointments with patients, and take telehealth into the homes for the most vulnerable patients.
