Project Description
Develop an integrated dental model for La Plata County, while maintaining the foundation SJBH has built in its three county area. This project proposes to improve access to comprehensive, quality and integrated oral health care for the underserved population of all ages in this rural region. San Juan Basin Health (SJBH) will work in partnership with Axis Health System and La Plata Integrated Healthcare (LPIH) on this project.
