Healthcare Incentives Payment Pilot

This project will continue to pilot the Prometheus payment model in two Colorado communities using a unique evidence-informed case rate to create incentives for coordinated, high-quality care while at the same time reducing potentially avoidable complications which account for a significant portion of the total cost of health care for chronic diseases.

Bridging the Gap

Increase the number of individuals who have access to adequate health coverage by targeting American Indian/Alaska Native populations with culturally appropriate outreach strategies and enrollment services.

Developing an Infrastructure to Enhance the Collective Impact of the Anschutz Medical Campus on the Health of Colorado (ECHO Colorado)

Support for a 10-month planning process to design a system to enable the Anschutz Medical Campus (AMC) to improve its collective impact on the health of Colorado. Building on the successful ECHO model in New Mexico, the AMC team will assess current efforts, community needs, and ideas of faculty, then design a framework that can effectively link the resources of the AMC to Colorado organizations and individuals to improve their capacities to improve population health.

Healthy Harbors

Provide a medical home to three underserved groups in Larimer County, Colorado through patient navigation. The target populations are: infants referred to the Department of Human Services because of parental substance abuse or family risk factors; foster/kinship care; emancipated teens or teens who are aging out of foster care.

Patient Navigation in Grand County

Continue the current patient navigation program and work toward program sustainability through the enhancement of evaluation and communication strategies.
