General Operating Support

Support health care safety net providers caring for low-income, underinsured and uninsured Coloradans throughout the state by strengthening key resources for clinics ensuring they are educated, equipped and prepared to be vibrant participants in their local health community. Communication, education and advocacy work will enhance our support for clinics to align their work with the Triple Aim.

Homeless Health Outreach

Grand Valley Catholic Outreach's Homeless Health Outreach program provides integrated health care to homeless individuals in Mesa County.

The Health Care Advocacy Program

Support adults with intellectual and developmental disabilities, including but not limited to individual and policy advocacy efforts, and the expansion of free health and wellness programs.

Fiscal Reform and Health Policy Impacts Project 2014-2015

Continue our fiscal reform and health policy research, analysis, outreach, education, and engagement efforts, including: Research and analysis of the state's fiscal challenges, including the impact of the aging baby boom population on the state's Medicaid system;
Outreach, education, and coalition work on next steps in fiscal reform; Research, identification, and preparation for the budgetary and programmatic impacts of health care and long-term care for the state's aging population; and
Support for effective implementation of health care reform in Colorado.

General Operating Support

Continued support for assisting, training and enrolling individuals into Supplemental Security Income (SSI) and Social Security Disability Income (SSDI) to include enrollment in Medicaid
